These scientifically recognized questionnaires are used in Alzheimer’s prevention research and help determine whether a medical consultation or memory assessment may be useful. The test takes about 10 minutes and is fully anonymous. At the end of the assessment, you receive personal guidance that you can share with your healthcare provider.
If your answers suggest that participation in a clinical study might be appropriate, we help connect you with nearby research sites. Participation is entirely voluntary, and your information is not shared with research centers unless you choose to contact them.

Stress, sleep, medications and other conditions can affect memory. The goal of the questionnaire is not to give a diagnosis but to provide clarity, reduce uncertainty and offer next steps.
